Cancellation Policy
- We understand that sometimes life happens, and you may need to step away from the practice for a little while.
- Cancellation of any plan simply requires a 30-day notice, and payments are discontinued on the due date of next billing cycle
- Refunds may be issued to prepaid annual memberships with a 30-day advance notice of cancellation
- Re-enrollment after discontinuing services requires an additional $100 reenrollment administrative fee, if space on patient panel is still available
